A Bit about the new Derma FNS

Fractional lasers create vertical channels into the skin maintaining the integrity of the skin surface, but stimulating dermal processes leading to skin tightening and remodelling.

Derma FNS™ uses multiple needles that vertically pierce the skin to create similar vertical channels which are designed to achieve the same effect, with minimal epidermal damage. It is thought that it is the greater density of channels that leads to enhanced results over dermal rollers.

Derma FNS™ uses vertical insertions to create thousands of tiny holes in the skin causing the release of serum growth factors which stimulate the breakdown of old fibrous collagen and stimulate new collagen and elastin development.

Derma FNS™ results are comparable in effect to ablative treatments such as Fraxel, IPL and Laser Resurfacing but at a fraction of the cost and with much less downtime and minimal damage to the integrity of the skin.

The Derma FNS™ is an electronically powered automated needling machine that delivers 120 insertions of nine tiny needles per second with over 70,000 insertions a minute. These multiple needles vertically pierce the skin rather than roll into the skin, as seen with a dermal roller. The dermal roller causes a v-shaped hole as the needles roll into the skin and then out again causing trauma. The rollers create holes twice as large as the Derma FNS and they are elongated further due to rolling mechanism. So we deliver more holes per square centimetre with intact skin inbetween and no scratching or catching for maximum results with minimum downtime.

our pens motorised mechanism pulls the needles out of the skin as well as pushing them in. All other pens motors on the market as far as we know push the needles into the skin but rely on a spring to retract them from the skin. These springs cannot keep up with a motor that pushes needles in 120 times a second, so they are not clear of the skin before they are reinserted, causing dragging in the skin.

Collagen production results from wounding of the skin, even though the wounds are miniscule. Each tiny wound goes through three classic phases of wound healing:

Inflammation
Proliferation (tissue formation)
Tissue remodelling

The needle only penetrates through the epidermis and does not remove it, so the epidermis is only punctured and puncture closes in minutes and will heal. The injury, minute as it might seem, does cause some localised damage and bleeding.
The needle pricks penetrate into the dermis and starts the inflammation phase.
Tiny blood vessels are ruptured so blood cells and serum get into the surrounding tissue. A complex chemical cascade determines the formation of a clot, vascular permeability, leucocytes and fibroblasts are recruited into the wounded area.
Platelets cause clotting and release chemotactic factors like Platelet Derived Growth Factor (PDGF),transforming growth factor and Fibroblast growth factor (FGF) that initiate an invasion of other platelets, leucocytes and fibroblasts.

The leucocytes, particularly neutrophils act on the damaged tissue, remove debris and kill bacteria.

This reaction is automatic and produces a surge of activity that inevitably leads to the

Fibroblasts being “instructed” to produce more collagen and more elastin.

Re-epithilialization occurs within a few hours after needling and this is mainly due to keratinocyte migration rather than proliferation. When the keratinocytes have reconnected together, they start producing all the components to re-establish the basement membrane with laminin, and collagen types IV and VII.
One or two days after the injury the keratinocytes begin proliferating and thicken the epidermis.
Fibroblasts migrate into the wound and produce collagen, proteoglycans, elastin and other matrix proteins.

Initially after needle damage, the disruption of the blood vessels causes a moderate amount of hypoxia. The low oxygen tension stimulates the fibroblast to produce more TGF, PDGF, and endothelial growth factor(VEGF).

Procollagen MRNA is also up regulated, but this cannot cause collagen formation because oxygen is required and that only occurs when revascularisation occurs.
Revascularisation occurs quite soon after needling.
TGF- is a powerful chemotactic agent for fibroblasts which migrate into the wound at about 48 hours after injury and starts producing collagen I and III, elastin, glycosaminoglycans and proteoglycans.
Collagen type III is the dominant form of collagen in the early wound healing phase and becomes maximal by 5 to 7 days after injury.
The collagen is laid down in the upper dermis just below the basal layer of the epidermis.
